Hello, I am a 20 year old woman with pcos. I have tried diets and exercise for years but I can’t lose my tummy which hangs down past my hips or my thighs which prevent me from wearing shorts when walking because of chafing so bad it puts me in immense pain. I’m not a fan of diet pills so what are my options? I’m 5’4″ and 240lbs.

The hanging skin and fat of the lower abdomen is called a panniculus. A panniculectomy means cutting away that hanging skin and fat and would solve your problem.
