Now more than ever, it’s critical to know your risk for Cardiovascular Disease

I’m concerned about being part of a high-risk group if I get the virus that’s turned our world upside down. And, at 73, my lungs and my heart could be even more vulnerable to harm if I had problems before COVID-19 became part of my vocabulary. While lung injury and respiratory problems took center stage as the most frightening complications of COVID-19, Cardiovascular Disease  is another. 

Alarming facts: About 10 percent of patients with pre-existing Cardiovascular Disease who contract COVID-19 will die, compared with only 1 percent of patients who are otherwise healthy, according to an article in the health blog from Harvard Medical School.  

Unfortunately, the absence of symptoms doesn’t mean you’re absolutely free of Cardiovascular Disease. This new viral infection, fever, and inflammation can destabilize heart vessel blockages you didn’t even know you had. This makes it imperative for anyone in the high-risk group to take ownership of your health and do everything you possibly can to avoid hospitalizations by eating right, exercising and getting screened for stroke and cardiovascular disease. 

Even without the risks associated with COVID-19, cardiovascular disease is the leading cause of death for men and women in the United States. Now that you’re moving around a bit more freely, one of the first tasks on your agenda should be to understand your risk for cardiovascular disease.

The carotid artery narrows and affects blood flow to the brain when it becomes clogged with plaque, and complications can be serious. If any of the plaque breaks off, it can block the blood flow to the brain completely, causing a stroke. Plaque in the arteries can develop over time without causing a singly symptom. 4) Abdominal aortic aneurysm, a bulging, weakened area in the wall of the aorta, which is the largest artery in the body. Over time, the blood vessel balloons and is at risk for bursting or tearing, which can cause life- threatening bleeding and potentially death. 5) Osteoporosis, a loss of bone density or mass.

Life Line Warned Me. I Didn’t Listen

I wasn’t overly concerned when I learned my right carotid artery had mild fatty plaque since it wasn’t a dire threat to my health, at least not immediately. Uncharacteristically, I dismissed the report recommendation to follow up with my doctor, too preoccupied with launching this website at the time. I felt good and had no signs of a problem. My weight was reasonable. I exercised. Didn’t smoke or drink. 

My blasé attitude had troubling consequences. 

By last year, my weight had spun out of control, I was working out less, and a new ultrasound showed a dramatic increase in the plaque. Now the bad stuff was narrowing the right carotid artery somewhere between 59 and 80 percent. That got my attention, even if there wasn’t a single symptom. If a piece of plaque broke off and blocked the blood flow, I could have a stroke. To make matters worse, my cholesterol numbers were dismal. My doctor started me on statin therapy right away to lower my cholesterol, told me to lose weight, start exercising again, and to take low-dose baby aspirin every day.  

Ironically, most doctors won’t recommend screenings for carotid artery or peripheral arterial disease for asymptomatic patients with no family history of stroke or heart attack. What’s more, Medicare doesn’t cover the tests unless someone has experienced symptoms. Without coverage, it’s costly to have tests like this done at a hospital. The United States health system isn’t a shining example of how to practice preventive medicine. 

Having no reason to suspect any cardiovascular problems a decade ago, I actually arranged for the first ultrasound through Life Line Screening as a preventive measure. When the original results showed mild plaque, it should have been fair warning not to let myself go. After all, that’s what preventive health care means. That’s what makes Life Line a smart idea.

When Life Line recently contacted me about introducing its screening service to my FOFriends, I didn’t hesitate. After all, they discovered the plaque in my carotid artery at an early stage, not to mention early bone loss, and suggested that I see my own doctor. Pretty foolish not to heed their advice. “Every day we find people across the country with significant carotid artery and cardiovascular disease,” said Dr. Keith Coffee, Chief Medical Officer at Life Line.

“We discovered about 62K overall health risks in 2017 alone,” Dr. Coffee said. A team of highly qualified, board-certified radiologists throughout the country, led by a vascular surgeon, reads all ultrasound images as well as the electrocardiograms for AFIB. 

“These are screenings, not diagnostic tests,” Dr. Coffee stressed. “We encourage patients to share their results with their doctors so they can together discuss the appropriate treatment, if necessary.” (Remember, that’s what I didn’t do!)  Life Line doesn’t have physicians available who can discuss the test results with patients, and it doesn’t interact with patients’ physicians. They do provide a summary of the results that you can share with your doctor. “We simply provide the incentive for patients to take the right steps for their health,” Dr. Coffee noted. 

If Life Line Screening discovers a life threatening condition, however, such as an aneurysm that’s about to rupture, it will send someone right to the ER and call for an ambulance if they wish. “Our customers tell us that we’ve possibly saved their lives. We get letters all the time from people who had no symptoms and thought they were just fine, but were at risk for having a large aneurysm about the rupture,” Dr. Coffee said.

In and Out In 90 Minutes Or Less

Life Line serves 60 cities across the country each day. Teams at the screening sites include at least two ultrasound stenographers who hold certificates in ultrasound technology, and two medical assistants who are cross trained to perform tests including electrocardiograms for heart rhythm, fingerstick blood tests, and peripheral pulses. A nurse practitioner joins most of the teams to see people 65+ years old for Annual Wellness Visits, which are covered under Medicare.

While most people opt for the basic, five-test package I described earlier, many decide to get their cholesterol and glucose checked during the same appointment, Dr. Coffee told me.  Life Line Screening offers 15 preventive screening blood tests, including thyroid function, vitamin D, and hemoglobin A1C for diabetes. It also provides a take-home test for detection of colorectal cancer. A call center with 300 trained employees helps participants decide what tests are right for them, based on their age, risk factors and medical history.

Testing is done in churches, at community centers, fraternal organizations and occasionally in hotel ballrooms. When Life Line works with affiliates, such as hospitals, anyone with abnormal results is referred back to the hospital. It also offers exclusive screenings for employees of large corporations as part of their healthcare plans. “Our goal is to get people in and out in 60 to 90 minutes, which includes paperwork and waiting time,” Dr. Coffee said. 

The Results Are In

A results package arrives in the mail about three weeks after your screening. It reports on every test you took and on your risk for stroke and cardiovascular disease in general, taking into account factors including smoking, high cholesterol and family history. “We give you all the information we can to help you make lifestyle changes and modifications if you need to,” Dr. Coffee explained. If a major issue such as AFIB is discovered, a Life Line representative will call you directly, rather than having you wait for the results package. 

People generally have their first Life Line screenings at 50 years old. “We have patients in their 40s who just want to know where they fit healthwise, or maybe they have a family history of early strokes. And, we have people in their 90s,” Dr. Coffee said. Returning patients account for about half of Life Line’s annual screenings, and close to 90 percent of all patients have their own physicians. 

Get preventive screenings once you’re age fifty, whether or not you have symptoms. And pay attention to what you learn.

Wrinkles, Dry Skin, Painful Sex, Oh My!

At this moment, yes, because the At-Home Health Monitoring service is associated with Medicare. Community Wellness also plans to offer the service to everyone under 65 shortly.

A high degree of tension always accompanies my mammogram appointments. My stress level has gone up, thanks to COVID 19. But I didn’t want to postpone the six-month follow up appointment for a 3-D image of my left breast since some tissue was suspect last time. 

The Perlmutter Cancer Center at NYU Langone Health in Manhattan seamlessly handled the new and more elaborate check in process. After registering at the front desk, I passed through a series of stations to pick up a mask, sanitize my hands and have my temperature taken. Everyone in the lobby respectfully stood at least six-feet apart from our sister patients.

Only one other woman sat in the third floor waiting room, usually packed with patients waiting for their mammograms, sonograms and other critical tests. A sign on every other chair instructed us not to sit there so that we could keep our distance from others. Within 10 minutes I was called to change into a dressing gown for the exam.

Technicians no longer congregate in a small room between patients, where they handle administrative work.  Each one now uses a computer right in the exam room. The technician who did my mammogram has been at her job since the start of COVID since it’s classified as critical. She’s periodically tested for the virus, she told me, but can’t be tested daily. Aside from arranging my breast in the imaging contraption – which takes seconds – she and I weren’t on top of each other. 

The radiologist told me that although new spots had appeared on my image, she wasn’t concerned because they’re probably due to the estrogen patch I use. She recommended that I return in six months for another follow up mammogram. When I started to automatically shake her hand and thank her, she chucked and gave me an elbow bump. “Oops, I forgot,” I said. 

It’s always impressive when organizations – and of course the people in them – adapt to adverse situations, devising systems and new behaviors that allow them to continue functioning successfully. COVID 19 has certainly forced us to hone our resourcefulness on many levels, professionally and personally.

Adapting to change can be difficult for many of us. I think of a middle aged woman I knew who completely shut herself off from friends after her husband died suddenly, and remained isolated from then on. He was her anchor, but once she lost him, she couldn’t find a new mooring. And a young man who had a devil of a time getting back on his career track after a debilitating accident. Sadly, he couldn’t adapt to his limitations; instead, he let them sideline him decade after decade.  

Contrast these two people to these two: A woman who loses her long-time husband, then moves to a new city, joins a vibrant community center and develops a new circle of friends. She also starts dating a couple of men from the group. And a man who almost loses his life after being hit by a taxi and is hospitalized for months, but fights his way back – day by day – all the while managing to keep his consulting company going. 

I recently heard the interesting term “lifequake,” which is an event that suddenly changes your life. Every single one of us experiences them, some more than others. Sometimes, we even cause our own lifequakes, say when we leave a secure job to start our own business or stop drinking after 40 years, even though it was such an integral part of our life. 

Millions of us are now going through the same lifequake. We didn’t ask for it, but just like a town that rebuilds itself after an earthquake, we must reconstruct our lives so we’re stronger than ever. 

Turning a Deaf Ear is Unsexy and Unwise

Are you one of the millions of women who color their graying hair and eradicate their frown lines, but passively accept hearing loss as an inevitable part of aging? I’m not. I hated pretending to follow conversations in noisy restaurants, or asking my friends over and over to repeat what one of the actors said in a movie. So, I added hearing aids to my accessories wardrobe about seven years ago, and I love them as much as my hair colorist and dermatologist. Please consider these facts from the National Institute on Deafness and Other Communication Disorders:

About 11 percent of adults aged 50 to 59 have disabling hearing loss, which increases to approximately 25 percent of those aged 60 to 69, and 50 percent in the population over 75 years old. 

About 28.8 million U.S. adults could benefit from wearing hearing aids.

Among adults 70 and older with hearing loss who could benefit from hearing aids, fewer than one in three has ever used them. 

This is one case where being in the majority can be a major mistake. A study of 1,984 volunteers in their 70s and 80s by hearing experts at Johns Hopkins revealed:

Older adults with hearing loss are more likely to develop problems thinking and remembering than those whose hearing is normal.

Levels of declining brain function were directly linked to the amount of hearing decline. 

Hearing loss often leads to social isolation, and it’s been proven that loneliness is a risk factor for cognitive issues.  

Hearing loss also affects balance and puts you at four times greater risk for falling.

You don’t have to make a full-fledged commitment to get hearing aids right this minute. But for goodness sakes, at least have your hearing checked by a qualified audiologist if you suspect you have issues. It first got my attention when I thought my hairdresser said I was wearing “autumn” colors in the dead of summer, although he actually said “Auburn” (as in the name of the university). Mistaking words that sound similar is a common sign that something might be amiss with your hearing. Hearing only half a word is another. 

There are about 12,000 licensed audiologists in the United States, so it’s likely you can find one near you. The hearing test takes about 30 minutes and is painless. Authorized Generics: What You Should Know

Debunking The 10K-Step Rule

I secretly congratulate myself when my Noom app shows I’ve walked 5K steps. If I manage to reach 7K, I’m tickled pink. 10K steps??? Not likely.

Putting one foot in front of the other 10,000 times is a cakewalk when I’m roaming the streets of Paris (will I ever see Paris again?). Otherwise, it’s no walk in the park. 

So when I discovered a Brigham and Women’s Hospital study on how walking impacts the long-term health outcomes of older women, I was ecstatic to learn that walking 10K steps a day isn’t the gold standard! FYI, the hospital is a teaching affiliate of Harvard Medical School “recognized internationally for its excellent and innovative patient care, biomedical research, and education and training programs for physicians, scientists and health care professionals,” says its website. 

Published in The Journal of the American Medical Association, the survey results of 16,741 (primarily white) women with a mean age of 72 years proclaim: 

Women who averaged approximately 4.4K daily steps had significantly lower mortality rates during a follow-up of 4.3 years compared with the least active women who took approximately 2.7K daily steps.  

And, as women took more steps each day, their mortality rates progressively decreased before leveling at about 7.5K steps a day. 

Translation: While walking at least 4.4K steps a day can help lower your risk of dying, you won’t fall apart if you stop stepping at 7.5K.  

While no one would dispute the health and longevity benefits of going the extra mile (assuming 2K steps per mile), it probably won’t further decrease our risk of mortality.

To measure participants’ steps, the study team required them to wear an ActiGraph GT3X+ accelerometer device — a research-grade unit — on their hips during all waking hours for seven consecutive days.  Over the 4.3 years the women were followed, 504 died. 

 ➠ Women who walked an average of 2.7K steps a day were at the greatest risk of death, with 275 dying. 

 ➠ Those who walked an average of 4.4K steps a day had a 41 percent lower risk of death.

  Risk of death continued to decrease with more steps walked, up to 7,500 steps per day, after which risk leveled off.

  Interestingly, the speed at which a woman walked – among all the women who walked the same number of daily steps – wasn’t associated with risk of death. 

The average American takes between 4K and 5K steps a day, previous studies show.  No one is sure where the 10K-step goal originated, but it’s been surmised that it started in 1965 when a Japanese company launched a pedometer called amanpo-kei, which means “10,000 steps meter” in Japanese.

Due to the observational nature of the study, the authors couldn’t definitively determine whether walking more lowers mortality or women in better health step more, reported the article in JAMA. Women with heart disease, cancer, diabetes, and less than excellent or good self-rated health were excluded, however.  What’s more, previous experiments do confirm the beneficial effects of  physical activity on short-term health markers,  including  blood pressure, insulin and glucose levels, lipid profile and inflammation. 

Further research in younger and more diverse populations is necessary to determine if the findings apply to other groups, especially to those who take more steps. The study also didn’t assess outcomes such as quality of life and risk of specific diseases. 

So, ladies, keep walking. But if you stop at 7.5K, or even 4.4K steps, you’ll still be doing yourself a world of good!
