There are multiple tissue tightening and firming technologies like Thermage, however the results have been mixed. About 50% of people who do get this are satisfied with the results. I currently like the Velashape, a combination of radiofrequency, vacuum, and light for the upper arms. You need to have weekly treatments for 8 weeks and come in once a month after for maintenance. However, it is gentle, even almost pleasurable and the results have been pretty consistent. This is one of the more difficult areas currently to get good results.

Sagging skin can often be a sign of hormone deficiency. In my practice I usually get a hormone panel to ensure that we are not missing any underlying problem.
There is a new device on the market that is similar to Thermage, but I have found it to be more effective and less painful. The device is manufactured by BTL industries and is called an Exilis machine. The results are not what you would get with surgery but there is no downtime and no scars.

One of the only procedures that really works is Thermage CPT, it is only effective if there is some sagging of the skin. For extreme sagging, surgery would be the best alternative but of course that will leave scarring.

I want to preface my answer by saying that there are no miracles for sagging arm skin in my experience, which is unfortunate for all of us women over 50. That said, the crinkling of the skin texture can be helped by AHA moisturizers. I have my patients use my glycolic acid body exfoliation kit at least twice a week. They use another rich moisturizer after their bath or shower on the other days. The skin looks better when the superficial crinkling is improved. This will not help actual sagging however.

There are skin tightening devices that can work for superficial skin tightening. Some people get nice results with this technology. They don’t lift sagging though. In my office we have the Skin Tyte machine. Skin tightening technology is evolving and nothing works for every person, results are very by individual.

Of course muscle strengthening of the triceps and weight management help this troublesome area. Lastly, there is always plastic surgery and liposuction, but I actually don’t have any experience with that nor can I think of any of my patients who have availed themselves of those sorts of procedures for the arm skin. I wish I had a better solution.
